In caring for children at Vineyard Columbus, we believe it is our responsibility to seek volunteers who are able to provide healthy, safe and nurturing relationships. Please answer the following questions accordingly. Understand that the information provided in this application may be reviewed by others on Vineyard church staff. Any concerns will be discussed individually with the VC Kids staff.

Applicant's Statement of Release

I hereby authorize the leadership of Vineyard Columbus to make any and all inquiries regarding my application and to verify all information contained in this application for a volunteer position in VC Kids. I also grant permission for any representative of Vineyard Columbus to share any information they believe may impact a decision for the volunteer position I am applying for.

I hereby authorize individuals to disclose any and all information to Vineyard Columbus. I release all such persons or entities from liability that may result or arise from Vineyard Columbus' collection of information or its consideration of my application. This release is in effect for 90 days from the date I submit my application to the VC Kids staff.

VC Kids Leader Commitment

Having committed to serve in the VC Kids ministry and to the habits essential for spiritual maturity and having attended a VC Kids Leader Orientation, I will commit to:

• Read and practice the safety and security policies, general evacuation and lockdown procedures of Vineyard Church.
• Prepare for ministry by growing in my personal relationship with Jesus Christ.
• Be a committed member of Vineyard Columbus—regularly attend weekend services, support the church with my tithe, and be involved in a small group.
• Be a committed member of the VC Kids team—support and pray for the VC Kids ministry, specifically the kids I serve.
• Maintain a teachable spirit as exemplified by a growing relationship with Jesus Christ, in my relationships with my ministry leaders and co-leaders.

Should my application be accepted, I agree to follow the policies of Vineyard Columbus and to refrain from unbiblical conduct in the performance of my services on behalf of the church. I certify the information and statements provided in this application are true and complete to the best of my knowledge. I understand my personal information provided will be held confidential by the Vineyard Columbus staff.
